Case Management
Nurses have the education, background, expertise, and knowledge of guidelines and standards needed to coordinate a patient’s healthcare services from onset of injury or illness to return to work or maximum functional ability.

Case management:
- assesses patient injury and illness, medical history, information of other healthcare providers, and treatment recommendations
- develops a treatment plan and expected outcome, based on assessment, in conjunction with the treating physician
- implements, monitors, and evaluates the treatment plan
- integrates communication and coordination of care among the patient and family, healthcare team, insurer, employer, and attorney to assure continuity
- educates the patient and family to maximize compliance and rehabilitation
- optimizes utilization of therapies and community resources
